Intraoperative pneumothorax identified with transthoracic ultrasound.

چکیده
P NEUMOTHORAX is an uncommon but potentially serious occurrence during anesthesia. Initial clinical signs may be subtle, but progression to a tension pneumothorax can result in significant cardiorespiratory instability. Because therapy for pneumothorax may require invasive chest tube placement, definitive diagnosis by chest radiography is necessary. However, intraoperative chest radiograph may be inconvenient and difficult, causing the diagnosis to be delayed until completion of operation. We describe two cases in which intraoperative transthoracic ultrasound rapidly established a diagnosis of pneumothorax and facilitated timely utilization of resources for definitive treatment.

A case of iatrogenic pneumothorax in which chest tube placement could be avoided by intraoperative evaluation with transthoracic ultrasonography
We report a case of iatrogenic pneumothorax in which chest tube placement was avoided by continuous intraoperative evaluation with transthoracic ultrasonography. A 53-year-old man had undergone a subsegmentectomy. While attempting to place a central venous catheter in the right internal jugular vein after the induction of anesthesia, we identified gas absorption during the puncture and suspecte...

متن کاملSonography of iatrogenic pneumothorax in pediatric patients
UNLABELLED Pneumothorax is defined as the presence of air in the pleural cavity. The incidence of iatrogenic pneumothorax in the pediatric population is 0.3-0.48 in 1000 patients. A conventional chest X-ray, in some cases supplemented with chest computed tomography, is a typical imaging examination used to confirm the diagnosis of pneumothorax. Within the last years, the relevance of transthora...

I N this issue of ANESTHESIOLOGY, Ueda et al. report two cases in which point-of-care ultrasonography led to the rapid intraoperative diagnosis of pneumothorax. In recent years there has been a dramatic increase in the utilization of ultrasound for real-time guidance of clinical decision-making and procedures. A growing body of evidence demonstrates the benefits of this change in practice. The ...
متن کاملTransthoracic needle biopsy for pleural and peripheral lung lesions: ultrasonography vs CT scan guidance.
It is well known that imaging-guided biopsy for pulmonary lesions will induce complications such as pneumothorax and bleeding. Preoperative enhanced CT imaging or intraoperative color Doppler ultrasound can be used to evaluate the vessel and avoid or reduce the operationrelated bleeding. However, the potential occurrence of pneumothorax makes us avoid performing CT scanguided biopsies. Th e occ...
